Your First Visit

If you’ve just scheduled your first visit with our team—welcome! To make your first appointment with us more convenient, our new patient forms are available online for patients to fill out prior to their first visit. If you would prefer to fill out your new patient paperwork in our office, please plan to arrive an additional 10-15 minutes earlier to have adequate time to do so. If you are planning on using your dental benefits, please remember to bring your insurance card and a valid form of I.D. with you as well. We look forward to your first visit with us!

Flexible Financing

Need a way to break up the cost of your care? Our practice accepts CareCredit payment plans, which can divide the cost of any dental treatment into manageable monthly payments. Most plans accrue little to no interest when payments are made on time, and credit decisions are typically made within minutes, so patients can begin using CareCredit right away.
